Main duties of the job
Undertake structured surgeries working in an autonomous role including the management of patients presenting with new acute problems or exacerbation of chronic disorders. To assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions.
To undertake home visits, primarily for acute conditions, as assessed by the duty doctor, and within the boundaries of professional qualifications.
Provide education to patients, promoting self-care and empowering them to make informed choices about their treatment.
Be a training and mentoring resource for peers and hosted trainees.
Promote the appointment system within the Practice, working towards a nurse-delivered same-day service supported by a Doctor for guidance and consultation.

Job responsibilities
The post holder will be an experienced Advanced Clinical Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
Demonstrate safe, clinical decision-making and expert care for patients within the general practice.
Perform as an autonomous practitioner working independently and with other members of the health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating direct referrals as appropriate.
Manage daily surgeries comprising face-to-face consultations, home visits and telephone consultations.
Manage results and workflow in line with practice policy and procedures.
Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Build effective working relationships with other health care professionals and will work within the team of General Practitioners and Nurses to deliver excellent primary health care services within the practice.
Person Specification
Knowledge and Skills
* Ability to work both independently and as part of a team.
* Good communication skills and computer literacy.
* Ability to self-motivate, organise and prioritise workloads.
* An understanding, adherence to the need for strict confidentiality.
* Ability to use own judgement, resourcefulness, common sense and local knowledge to respond to the needs of patients.
* The ability to respond positively to changes both within the practice and the wider healthcare system.
* Hold a current driving licence and have the use of a car.
* Competent with the EMIS clinical system.
Qualifications
* NMC Registered Nurse
* Completed MSc in Advanced Clinical Practice
* Level 7 V300 Non-Medical Prescribing or equivalent
* Completed a minor ailment and injury module at BSc/MSc level
* Evidence of PREP
Experience
* Several years post registration
* Experience of working in general practice
* Experience in managing long-term conditions
* Experience in management of diseases and undiagnosed health conditions
* Experience in minor illness, clinical decisions, telephone triage
* Experience of working autonomously
* Excellent interpersonal and communication skills
* Ability to work effectively in a multidisciplinary team as a key player
* Triage/minor illness work, A&E or as an Emergency Care Practitioner
