*Hours:* Part-time – Mondays, Tuesdays, and Fridays (Friday can be a half day)
*Salary:* £23–£24 per hour (Flexible) / £46,000–£50,000 FTE

We are recruiting on behalf of a single-site GP training practice in Hersham, seeking a dedicated *Diabetes Practice Nurse* to manage the care of their diabetic patient population. This role is perfect for an experienced nurse or someone interested in specializing in diabetes, as training and a comprehensive handover period will be provided.
*Key Responsibilities*
The nurse will take charge of managing the care of approximately 450 diabetic patients. Key duties include:
* Diabetes management and education.
* Collaborating with GPs and the multidisciplinary team to deliver high-quality patient care.
