A fantastic and exciting opportunity has arisen within Central and North West London NHS Foundation Trust for a highly motivated and enthusiastic Band 6 Community Mental Health Nurse to join our Hillingdon Older Adult Home Treatment Service (HTS).
The post is full-time, 37.5 hours per week across 7 days and the service hours are Monday to Sunday 9am-5pm. We also have flexible working policies to support individuals achieve the work-life balance that their situation might require.
The HTS is a needs-led service and works with service users that are in crisis, are physically frail and have a mental health problem. Caseloads are a mixture of service users with long and enduring mental health issues, acute mental health issues or issues relating to memory loss who are in crisis.
The HTS is a multidisciplinary team comprising mental health nurses, a psychiatrist, and a support worker. The team is supported by a good administrative system and offers an excellent work environment. The team also works closely with our colleagues at OACMHT, the older adult acute inpatient ward, and the wards within the Riverside Centre at Hillingdon Hospital and the Memory Service. The team has good partnership working with local non-statutory Dementia services.
Main Duties of the Job
The role will focus on triage, assessment, and care planning for older adults in Hillingdon who have been referred into the Crisis pathway. Working as part of the Older Adult Home Treatment Service, the role offers intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care. The role will also act as gatekeepers to admissions onto the acute inpatient ward and support early discharge from the hospital.
The HTS works with older adults covering the Hillingdon borough and is based in the Woodland Centre in Hillingdon. Hillingdon is home to a diverse population and contexts, from urban to semi-rural. Situated in North West London, it has good transport links with the rest of London and surrounding counties and is home to a wide range of social, leisure, and sporting opportunities available to people working and living in the area.

Job Responsibilities
What we are looking for?
1. Post-registration experience working with adults/older adults with mental health problems and experience of working with adults in a community setting and/or crisis service.
2. Excellent communication and interpersonal skills, with a sound understanding of current government legislation, demonstrating the ability to work as part of a MDT and provide high-quality care with evidence-based therapeutic assessments, interventions, and completion of accurate and timely documentation.
3. Specialist skills in mental health assessment, risk assessment, safety planning, and social care assessments.
4. Car owner/driver would be ideal but we can work around this within the team.

Qualifications
* RMN-Diploma/Degree
* Evidence of continued professional development in post-registration qualifications
Experience
* Post-registration experience in a range of clinical areas
* Experience working as a member of a multidisciplinary team
* Experience managing/supervising staff
* Experience working in a community setting and/or crisis service
* Experience working with dementia
* Experience working with older adults
Knowledge, Skills, and Abilities
* Knowledge of CPA, Risk Assessment, and Risk Management Processes
