Key Responsibilities Provide a single point of access to community intermediate care services, including reablement and residential rehab Support hospital discharges and work with the wider multi-disciplinary team to prevent hospital admissions. Utilize the Discharge to Assess model for assessments away from the hospital environment. Conduct holistic, face-to-face Care Act Assessments, reviews, and support planning using a strengths-based approach.
