To keep the patient at the centre of his or her care, ensuring patient and carer parti Working within a multi-professional team to provide delegated care. • Delivering treatment and support as outlined in a care plan under the guidance of a registered practitioner. • Keeping patients at the center of their care, ensuring active participation from both the patient and carers in decision-making. • Supporting daily care needs for assigned patients in their home environments. • Providing treatment and monitoring of patients as per care plans, following established guidelines and protocols. • Reducing unnecessary hospital admissions and supporting early discharge by providing timely and appropriate care. • Maintaining clear, concise, and accurate documentation within an electronic patient record system. • Recognizing deteriorating patient health and escalating concerns to the appropriate registered practitioner.
