We are looking for 2 x Band 7 Clinical Team Leaders to work as In Reach Discharge Coordinators facilitators. Each of the coordinators will be aligned to one of the teams in Guildford and Staines.

The CRHTT Discharge Facilitator will support the Trust's Home Treatment Teams in managing patient flow from acute inpatient wards into the community by facilitating early discharge from the inpatient units. Ensure all bed capacity across the Trust is utilised whilst ensuring patients experience safe, high quality care throughout their patient journey.
You will work with system partners and colleagues to expedite safe transfers and provide safe alternative pathways for patients. You will work collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised and patient flow is maximised.
Main duties of the job
This job makes better lives by providing a robust in-reach service into inpatient wards by attending ward rounds/board rounds to discuss those who have reached their expected criteria for discharge (ECD).
You will confirm and challenge purpose and goals of admissions and where these are met will be looking to support in community. Where purpose and goals of admission are not met will be asking for immediate review of treatment.
You will also identify opportunities for early discharge as soon as someone is admitted and if there are any barriers to their discharge. You will proactively prepare people for early discharge by reassessing their needs, mental state and risks and feeding back to the MDT to discuss their identified needs should they be discharged early from hospital.
You will communicate with field HTT and community colleagues once you have undertaken the initial assessment of those ready for discharge. You will also confirm and challenge where they are identified delays beyond expected date of discharge.

Person Specification
Qualifications
* Registration with NMC/HCPC/SWE
* Clinical qualification at Diploma level or above in Mental Health nursing, with evidence of transferable mental health skills and practice.
Experience
* Car driver and access to vehicle
* Experience working in HTT
* Experience in managing or leading a clinical service.
* Cognitive Behaviour Therapy
* Non-medical prescribing
* ENB 998 Teaching and Assessing Students, Supervising and assessing students
