The purpose of this post is to provide advanced, specialist, clinical leadership to the countywide cardiac service, prioritising quality and safety of care provision, minimizing incidents and risk and ensuring a robust and resilient management structure is maintained.
To work alongside the operations Manager to ensure service spec, quality standards and KPI's are achieved by the service.
Leading on transformation and projects to optimise service efficiencies and productivity, analysing data and working with the performance team and clinical effectiveness teams.
Lead on clinical governance for the service with better integration of cardiac rehabilitation and the heart failure service in alignment with the five-year plan.
Act as a resource for the professional and clinical development of CPFT staff within this specialist service and across other CPFT services.
To represent CPFT at the regional and local cardiac networks, advocating for patients managed in the community with complex cardiac conditions.
Main duties of the job
To maintain up to date knowledge of legislation, national and local policies and issues in relation to both the specific client group and mental health.

Job responsibilities
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. * To hold a clinical caseload, with approximately a 70:30 split of time clinical patient facing: management
* Work collaboratively with the Cardiac Service Operational Manager to ensure all staff appropriately manage and support individuals within our service, utilising knowledge, and advanced skills to assess, plan, implement and initiate changes where required.
* Provide expert clinical advice to the team to support complex case management, clinical decisions, and staff reasoning.
* Work within the model of the Older Persons and Adult Community Directorate, providing leadership and management on the matters relating to admission avoidance, early supported discharge, planned care and transfer of care in conjunction with other services.
* Ensure own leadership style facilitates effective communication, collaboration and motivation of staff and partners to achieve effective function of the Cardiac service model to respond to National Policies and Clinical Guidance.
* Work collaboratively with neighbourhood teams, professional leads and GP practices to support professional and practice developments.
* Be proactive in developing own professional practice, demonstrating evidence of autonomy, judgement and decision-making skills.
* Lead the on-going development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols and guidelines
* Work alongside the operational manager to represent the service in relevant forums
* Ensure the provision of high quality, responsive services with the need of the patient at the centre of service delivery
Person Specification
Education and Qualifications
* Registered General Nurse or Allied Health Professional with cardiac interest
* To hold a non-medical prescribing qualification V300
* Experience within heart failure acute and/or community
* Experience within cardiac rehabilitation
* Educated to degree level or equivalent professional experience within NHS or social care or private sector
* Masters' degree qualification, or working towards a Masters' degree
* Experience in organisational development and change management
* Cardiac experience at manager/senior level
