Main duties of the job
The role involves assessing eligibility criteria under the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care (revised 2022), planning and commissioning packages of care (including Personal Health Budgets), supporting complex individuals, their families, and health professionals through the NHS Continuing Healthcare process (including transition from child to adult services) and reviewing eligibility/packages of care.

NHS Cambridgeshire & Peterborough is an Integrated Care Board (ICB) and the statutory NHS organisation responsible for planning and delivering local health and care services to the population and communities of Cambridgeshire & Peterborough.

Job responsibilities
We need a registered professional practitioner who is experienced working with people who have complex physical and mental health needs. NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital, but who have complex ongoing healthcare needs.
The Complex Cases Team is responsible for the decision-making and commissioning processes, working closely with the Local Authorities throughout Cambridgeshire and Peterborough.
Successful candidates will have excellent communication and assessment skills; be resilient, able to work autonomously but also work as part of a team. This is a home-based post role, and post holders will be expected to be able to travel across Cambridgeshire and Peterborough.
The team values a role which is fast-paced and employs the use of holistic nursing where we look at the needs of the person. The nature of the role is autonomous and challenging but with the benefit of supportive colleagues.

Person Specification
Qualifications
* Educated to degree level or appropriate equivalent experience. Registered Nurse - General/Adult/Mental health/Learning Disability or Allied Health Care Professional (Occupational therapist/physio therapist, speech and language therapist). Valid Professional registration.
* Continuing Care Specialist training or qualification. Mentorship Qualification.
Skills, Ability & Knowledge
* Awareness of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care. Knowledge and understanding of NHS and Social Care Services structures and Independent Sector Organisations. Knowledge of the Care Act 2014 and its implementation. Fully conversant with the assessment process and need to assess, plan, implement, and evaluate the needs of clients.
* Detailed knowledge of National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and its implementation.
Experience
* Experience of carrying out complex nursing/healthcare assessments for clients with health and social care needs. Experience of care planning for clients with complex health and social care needs. Experience of working in partnership between health and social care. Demonstrable based nursing experience Vulnerable adults legislation.
* Experience in a nursing role within NHS Continuing Healthcare/NHS-funded Nursing Care. Experience of assembling specialist packages of care which are tailored to the needs of individual clients. Experience and awareness of budgetary issues ensuring concordance. Experience of delivering training programmes to groups. Proficient in the use of IT systems.
