More detail about the role This high profile service which has developed and expanded significantly over the previous five years delivers short term crisis response activity in peoples homes. It is at the front line of driving the national agenda of avoiding unnecessary hospital admissions with high profile and demanding activity and performance targets that are scrutinised at a strategic level. This service has significant senior clinical leadership and interest from clinicians across the Trust. The existing skill mix of nurses, physiotherapists, occupational therapists, paramedics, support workers and coordinators has the potential for further development and expansion in partnership with other services. Collaborating with clinicians and coordinators will be crucial to bring success to the role. The focus for the role will be to ensure that there is equity of service across Northern and Eastern Devon, enhance integration of community based services with acute and primary care services including virtual ward and acute admission avoidance, ensuring consistent utilisation of the teams and to develop new pathways to maximise the opportunity of keeping people at home and avoiding unnecessary hospital attendances. The ideal candidate will have proven experience and an ability to work at this level. The successful candidate will be comfortable getting to grips with operational detail and problem solving at an operational and strategic level. The post holder will represent the RDUH at system wide meetings with the ICB and be responsible for driving the strategic development of the service.
