This post holder would be based in Bath at the Royal United Hospital (RUH). The main focus of the job involves working with the RUH Discharge services in an integrated model to ensure safe and timely discharges for all Wiltshire in-patients in the hospital. As a clinician you will be coordinating discharges, deciding Discharge pathways and working closely with the Wiltshire Flow Hub as well needing to liaise with services both internal and external to the hospital. Main Responsibilities • Ensure a consistent presence in the hospital enabling involvement and opportune interactions • Promote continuity of care for patients moving between services • Support a patient focussed approach to care, promoting respect and dignity • Proactively seek to support reductions in average length of stay • Attend ward whiteboard meetings as required to support ward teams to maintain focus and progress on discharge planning, supporting in decision making process • Participate and lead as necessary on reviews and meetings where critical decisions are made in relation discharge planning • To inform and update the acute hospitals on organisational discharge policies and national guidance. • Have a clear understanding of Mental Capacity Act and the impact this may have when planning discharge • Hold an acute awareness of potential safeguarding concerns and the correct route to escalate these • Have a detailed understanding of Continuing Health Care funding and the process for assessment • Maintain oversight of Wiltshire discharges to ensure all elements of discharge are considered, including medication supplies, equipment ordering onward referrals as example • Act as a support and resource for both patients and their carers, developing effective channels of communication • Ability to flex communication style to the needs of others. The Ideal Candidate • Registered Clinician, or experience of working within health or social care discharge planning role • Ongoing evidence of continuing professional development • Experience of working across organisational boundaries within health and social care • Knowledge and understanding of current legislation and best practice in relation to key areas including Information Governance, Care Act, Mental Capacity Act. • Up to date understanding of National Hospital Discharge policies and guidance • Ability to manage, and adapt to change within multiple teams • Ability to contribute to, monitor and implement changes and improvements to services • Knowledge of safeguarding policies and procedures • Ability to interpret clinical information to support risk assessments when planning discharges • Excellent communication skills, both verbal and written, and the ability to influence others in clinical decision making towards discharge • Able to change priority, often at short notice • Creative approach to system pressures • Sound conversational skills allowing for difficult conversations with both patients, families, and staff • Ability to remain calm and consider wider picture in particularly stressful situations. • IT proficiency • Ability to prioritise workload, often in response to a changing picture • Ability to work autonomously as well as part of multiple teams • This post will require onsite presence at an acute hospital About The Company We change lives by transforming health and care.
