We are seeking an experienced nurse to support the Specialist Palliative Care Nursing Team to provide direct palliative care to a caseload of patients. You will be responding to unplanned events and working alongside the Specialist nurses and wider community colleagues to provide palliative care support to patients within Bedford.
Experience in the field of adult palliative and community services is desirable.
We are looking for someone who is dynamic and motivated and has the confidence to support and develop individuals to deliver consistently high-quality services. Excellent communication and time management skills, and sound clinical knowledge, are essential. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.
Main duties of the job
This is an excellent opportunity to develop your knowledge and skills in Palliative care. You will work closely with the team to support and facilitate end-of-life care at home. You will also have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will be responsible for a specified group of patients, with the support of the Specialist Palliative care Nurses and respond to calls within Urgent Community Response to ensure people are supported to remain at home and to prevent unnecessary hospital admissions.
This is a 0.6 WTE part-time post to work 3 days a week 9-5pm.
You will be aligned to the Specialist Palliative Care team who work from Bedford.

Essential
* Registered General Nurse
* Palliative Care experience
* Current driving licence and the ability to travel across Bedfordshire to meet the needs of the service.
* Ability to travel across sites and across Bedfordshire to meet patient needs and attend meetings, etc.
* Post registration qualification in a relevant field
* Demonstrate evidence-based practice and up-to-date clinical skills
* Evidence of and commitment to multi-professional working
* Knowledge of the principles of Safeguarding of Vulnerable Adults and Children
* Comprehensive understanding of partnership working
Desirable
* Community Nursing Experience
* Experience of managing a caseload
* Literate in IT / Computer Skills
* The ability to collect and collate data
* Undertake the moving and positioning of patients and equipment adhering to the manual handling policy when in clinical practice.
* Holds qualification to teach and mentor others
* Understanding of safeguarding and essentials of managing complex cases
* Experience of audit and review of care
* Deliver high-quality evidence-based care in a variety of settings some of which may be unpredictable or undesirable.
