We are looking for a person-centred Social Worker to join our award-winning Urgent Care team. This is an exciting time to join the Community Crisis Response Team as investment allows expansion of roles in admission avoidance and the development of exciting new initiatives. All roles are valued members of this highly integrated team and you will gain great satisfaction in using your skills, in collaboration with trusted health and social care colleagues, to work with patients during their health crisis.
The team includes Physiotherapists, Occupational Therapists, General Nurses, Mental Health Nurses, Social Workers, Community Care Workers, Paramedics, support workers and coordinators. In addition, there are opportunities for working with Acute Response Practitioners within the team to expand your understanding of physical assessment and clinical reasoning of frail elderly patients in primary care.
CCRT promotes a culture of learning and improvement, and as part of our team you will have the opportunity to gain desirable interdisciplinary skills whilst maintaining your social care identity. Staff are supported with a regular training programme to meet team competencies, as well as encouragement to attend relevant professional courses.

Main duties of the job
* Manages a day-to-day case load and will be required to assess and support people to live independent lives in the context of intermediate care criteria and statutory duties in order to enable vulnerable people to manage their own lives.
* The post holder reports to the Advanced Practitioner via annual performance appraisals and six monthly reviews, 1:1 meetings where case work can be discussed as necessary and regular professional supervision.
* Responsibility for holistic and partnership assessments of service users and their carers who have complex needs, utilising Social Work theories, personalised approaches models and methods to identify strengths and needs to inform appropriate interventions.
* Assess the urgency of a request for action and identify the need for legal and procedural intervention.
* Complete assessments through detailed conversations with all groups of clients that may include those with mental health illnesses, learning disabilities, substance and alcohol misuse, sensory impairment, acquired brain injuries and a wide range of physical disabilities.
* Assess the outcome of any intervention against need, professional standards and best practice under the Care Act 2014.
* Assess eligibility and negotiate immediacy of intervention against resources, risk and need. The post holder will need to have an awareness of budgetary constraints.

Person Specification
Qualifications
* You must hold a qualification in Social Work, such as postgraduate diploma, bachelor's degree or master's level.
* You must have a valid Social Work England professional registration.
* Any postgraduate qualifications relevant to social work, such as Graduate Certificate in Practice Education or Approved Mental Health Professional.
Experience
* Proven relevant experience of working in the health and social care sector.
* Experience of working with other professionals in an integrated service.
Knowledge and skills
* Knowledge of social work theories and models, the strengths and limitations and how they can be applied to practice.
* Knowledge of the legal framework relevant to social work, including the Care Act (2014), Mental Capacity Act (2005), Mental Health Act (1983) and subsequently in 2007; and The Human Rights Act (1998).
* Knowledge of the Professional Capability Framework and the standards set by the social work regulatory body, and maintaining professional registration.
* Undertake safeguarding investigations and the application of the 6 principles of Making Safeguarding Personal.
* Ensuring that case records are accurate, timely and that work is completed in accordance with the priorities outlined by the line manager; and record keeping guidelines.
* Knowledge of total communication (e.g. BSL, PECS and Makaton). Experience of writing court reports and/or appearing in court.
Other requirements
* To be available to work in a service that operates 7 days a week, with varied shift patterns.
* Hold a full UK driving licence.
