An exciting opportunity for a District Nurse has arisen within the Bury Town (Fixed Term) Integrated Neighbourhood Team. Bury Town is one of six INT teams in West Suffolk. Our other INT localities are based in Newmarket, Bury Rural, Mildenhall, Haverhill and Sudbury.
The INTs are the base for a wide range of other health professionals and agencies including Adult Social Care, social prescribers, and mental health team members so we are able to work together in a fully integrated way to ensure the best care is delivered to the patients within our localities.
The successful applicant will join a dynamic, friendly team of community nurses, community matrons, therapists, social prescribers, social care staff, INT coordinators and local administrators, supported fully by a locality Team Manager, and Localities Lead.
Main duties of the job
You will be supported with a comprehensive induction and training, as needed, to help you settle in, with the time & opportunity to get to know the people on your caseload, along with development opportunities.
This post requires a specialised District Nursing degree, following your Adult Nursing qualification. Community Nursing experience is essential.

Job responsibilities
1. Manage a caseload of complex patients with healthcare needs within a designated population, using specialist knowledge to carry out assessments.
2. Assess, develop and implement individualised and specialist nursing care programmes in the community.
3. Evaluate treatment interventions for the caseload and modify healthcare plans. Encourage the patient towards self-care and independence, using a health coaching approach and to provide training, education and advice.
4. Work in partnership with other services to prevent inappropriate admission to acute hospital settings.
5. Support discharges into the community.
6. Effectively manage and oversee the triage of all referrals to the team.
7. Ensure all aspects of care offered by the team meet the agreed National, Professional and Local standards.
