Job summary The main duties of the posts are to assess and review care for individuals with complex health care needs within the community and care homes. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Main duties of the job An exciting opportunity has a risen for a band 7 post within the CHC & Care Home Governance Team. The successful applicant will have experience across the areas of CHC, safeguarding and care home governance. The position will be aligned to a locality Senior Nurse, and work collaboratively and flexibly with other localities where there are increased work demands, as part of the wider developments to manage the workflows more equitably across these areas of work. This post holder would be responsible for working jointly with health, social care and independent sector colleagues, and have a key role with monitoring the contracts with care homes and quality of patient safety and the safeguarding agenda; providing professional advice, support, guidance to professionals and families concerning all aspects of CHC. Vital to this appointment are excellent communication and interpersonal skills, working collaboratively with team colleagues, managing priorities of a case load, having excellent analytical skills to interpret information, preparing reports of a high standard that are understood by the person/people who they are written for. The role will be based at Ty Gwent in Cwmbran, but you will be required to travel to other parts of the Health Boards services, care homes and local authority or other service areas.
