Cornwall Partnership NHS Foundation Trust
We are seeking an Advanced Clinical Practitioner to join our medical team at Camborne & Redruth Community Hospital.
You will work alongside our GPs & ACPs assessing & managing patients presenting to our Same Day Emergency Care unit (SDEC) & those who have been admitted to Cornwall's Virtual Ward.
SDEC provides an alternative to the acute hospital for patients who can be safely managed outside of the emergency department with the aim of home on the same day. The follow-up is provided by our Virtual Ward. Patients are kept at home with regular nursing contact & medical oversight 7 days a week. This is a Consultant-led service with opportunities to join ward rounds & MDTs for learning & service provision.
We also host a Urgent Treatment Centre & Community Assessment and Treatment Unit (CATU). The post holder may rotate through these services depending on skill & service requirements. The UTC sees a range of medical problems covering all age groups, self-presenting to the unit. The CATU provides short-stay, ward-based medical and nursing care, with therapy assessments.
The role will be varied and challenging but well-supported by our existing GPs. There is time and scope for leadership, personal development, and project opportunities. The role can include bespoke creation while supporting us to deliver safe medical cover to our patients across these services.
This role will suit an experienced ACP, comfortable with working independently.
Main Duties of the Job
1. History-taking, examination, requesting investigations and creating an appropriate safe medical management plan for patients presenting to the services above.
2. Request and interpret investigations such as blood tests and x-rays.
3. Prescribe appropriate medications to support the patient's care.
4. Utilise all alternatives to admission services to enable people to be treated in their own homes whenever possible.
5. Work closely and collaboratively with your team of the day, with a 'can-do' 'yes' approach to your working.
