Following promotion, an exciting and unique opportunity has arisen for to join the Colorectal Clinical Nursing team coordinating the care of colorectal patients.
The post holder will:
• provide specialist nursing advice to a cohort of patients referred for specialist intervention, care and support within the colorectal services, being a point of contact for assessment, information and advice for the patient, family and carers and all staff involved in the care the patients
• in liaison with others support the development of staff caring for patients through support, formal education, practice development and role modelling.
• deliver all aspects of care relating to their patient group. Including elective and emergency admission patients. Care elements will include counselling patients pre and post operatively, educating patients and their carers in stoma care and empowering the patient and relatives to self-manage the stoma to achieve a safe, timely discharges.
This post is available for secondment/fixed term for a period of 12 months.
We are looking for someone who is ready to develop their clinical and leadership skills, who has experience in a higher-level nursing post and is keen to hit the ground running to ensure we can make the most of this opportunity.
• To be a competent practitioner, leading innovation and demonstrating clinical expertise. The postholder will act as a resouirce and advisor in their area of expertise to colleagues in other wards and departments through the Trust.
• to provide appropriate clinical assessment/ care and advice for patients with large exudating wounds requiring use of wound containment products, liaising with the senior colorectal CNS and tissue viability CNS where appropriate.
• To ensure that stoma care/colorectal information is available and provide specific advice and support to patients with benign disease and cancer, this will often include communicating sensitive information about diagnosis, treatment options, prognosis and outcomes of illness including future care, terminal illness and bereavement
• to participate in the management of the colorectal/stoma care telephone support service along with offering support via telephone for pre/post cancer treatment patients and their carers.
• Participate in multidisciplinary clinics, ward rounds, patient reviews and multidisciplinary team meetings. Undertake and provide treatment / advice as per speciality and scope of practice. These may be face to face, virtual or telephone, and incorporate the use of agreed protocols clinical and professional guidelines. This service may be provided within an acute secondary health care facility or in a primary, community or home care settings.
You will join the team of eight experienced nurses (a combination of Band 7 and Band 6 colleagues) who comprise the current Colorectal CNS team and a further team of Project Manager, Lead Nurse, Clinical Nurse Specialists and Secretarial support based at Hollinswood House.
