Phyllis Tuckwell provides supportive and end oflife care for people living with an advanced or terminal illness. PhyllisTuckwell is based in Farnham, Camberley and Guildford and serves a populationof 550,000 across West Surrey and NE Hampshire.
We have an opportunity for a Clinical Nurse Specialist working within one of three localitieswith flexibility to work across our catchment to respond to patient needs. The three localities are: Guildford, and Waverley,Surrey Heath and Farnham or North East Hampshire.
Salary £49,400 WTE based on 37.5 hours per week
Full and part time opportunities available
Full time hours - worked across 4 days of 9.5 hours or 5days of 7.5 hours on a seven day rota.
Part time hours - worked across 2 or 3 days of 9.5 hoursor 4 days of 7.5 hours on a seven day rota.

We are looking for an experienced senior nurse withexpertise in palliative and end of life care, clinical leadership andpartnership working, to join our community team to provide specialistpalliative care to patients, their families, and carers in the community.
Main duties of the job
The Clinical Nurse Specialist role will beresponsive to need, demonstrating advanced communication skills, advance careplanning, symptom management knowledge, caseload management, as well ascritical thinking and decision-making skills.
The role will include working in our Advice andReferral Team which provides a single point of access for referrals, advice,and support for multidisciplinary team working around the patient.
The successful candidate will work as part of amulti-professional team and will provide responsive care as well as proactivelyworking alongside patients and families to support them to live well.They will embrace opportunities to advance clinical practice, be willing toshare their skills and knowledge by educating others and be committed tocontinuously improving services. A commitment to further develop ownknowledge and clinical skills to improve patient care is essential including awillingness to undertake MSc modules in Advanced Clinical Practice.

RESPONSIBILITIES:
1.0 Clinical Leadership
1.1 Toprovide clinical leadership through the provision of evidence based expertclinical practice.
1.2 Toundertake assessments, plan, implement and evaluate palliative careinterventions and support to the patient and family or carer within thecommunity setting.
1.3 Toprovide expertise and support on pain and symptom management and strategies toenable psychological health, through advanced communication.
1.4 Toliaise with the GPs, District Nurses, other health care colleagues and membersof the primary health care team to ensure effective continuity of careirrespective of the care setting.
1.5 Toprovide effective decision making and proactive caseload management, arrangingfor appropriate cover when on leave for continuity of patient care.
1.6 Use advanced communication skills tonegotiate, influence and ensure best practice takes place focusing on the needsof the patient.
1.7 Prescribewhere appropriate and within own competency, in line with PT policy.
1.8 Participate in a 7-day a weekservice across the area covered by PT, demonstrating flexibility and rotationinto the Advice and Referral team.
1.9 To utilise skills in coordination,prioritisation, delegation, and caseload management to work with RegisteredNursing and other colleagues in the locality to ensure that individual patientsand the whole caseload have equitable access to care.
2.0 Professional
2.1 To act as an ambassador for PT on alloccasions in the general public arena, for example by giving educational talksand to ensure that the reputation of PT is maintained and enhanced.
2.2To ensure patient records areproperly maintained in accordance with NMC guidelines and PT policies.
2.3 To abide by professional NMC registrationrequirements for practice and revalidation.
2.4To develop an area ofexpertise and act as a resource for the multi-professional PT team, advancingclinical practice.
2.5To participate in and contribute to meetings, andprofessional development.
2.6To maintain appropriateprofessional relationships and partnership working with other health and socialcare agencies, respecting and valuing the role of other professionals.
3.0 Managerial
3.1 Be familiar with all guidelines andregulations relevant to the role at PT, in the hospice, at the Beacon Centreand in the community, particularly those relating to governance, lone working, healthand safety at work, ensuring they are met at all times.
3.2 Ensure patient safety and a risk managementapproach underpins the role.
3.3.Report accidents, incidents and complaints in area ofresponsibility as per PT procedure, participating in investigations asrequired.
3.4. Contribute towardsachieving contractual and organisational KPIs.
3.5 Participate in audit/s of the service,to identify if improvements and/or changes have enabled improved patient andcare experience
