Main duties of the job Redwood Unit is fundamental in the delivery of the health boards' clinical futures model in providing care closer to home and a key part of our community team family within Caerphilly borough locality. We deliver holistic care to patients through appropriate assessment and planning of nursing and therapeutic interventions supported by our therapies and social worker team. The onsite GP's hold medical governance of patients and we work closely with them to ensure patients receive quality standards of care. Delivering high standards of care are achieved through effective collaboration and partnership working within our health and social care integrated centre at Rhymney and wider primary care and community services. The centre houses Redwood Community Hospital, social services, the district nursing team, GP surgeries and many other health and social care professionals, thus, enabling a collaborative way of working and delivery of effective and timely care to patients.
