An exciting opportunity has arisen for a band 7 Heart Failure Specialist Nurse to join our community based service.
We are looking for a dynamic, highly trained nurse to join our well established service that covers Leicester, Leicestershire and Rutland. Our community based specialist service works to improve outcomes for patients living with heart failure.
By working as part of a wider MDT we strive to deliver best evidence pharmacological and holistic care that is individualised for our patients. We work to reduce hospital admissions and associated costs, improve quality of care, quality of life and the patient experience. We support people with heart failure to self manage their condition and bridge the interface between primary and secondary care.
You will see patients in community based clinics and in their own homes and therefore a full UK driving licence is essential.
We are a 7 day service that covers a virtual ward at the weekend.
You will actively participate in the development of the service and take responsibility for supporting and teaching other staff.
Main duties of the job
* Work as the Specialist Nurse within a locality within the Long Term Conditions Team, liaising with all health care professionals across organisational boundaries for the benefit of the patient and be responsible for the management of care and service provision for a disease specific pathway within the locality.
* Proactively deliver and manage a case management service in a community setting, to a defined population of patients on a locality caseload, working with other health care professionals.
* To recognise and respond to the early symptoms of disease exacerbation and acute illness based on the understanding of the chronic disease, the disease process, including current evidence and practice standards.
* Providing care closer to home and preventing inappropriate hospital admissions.
* To deliver pharmacological expertise to ensure patients receive best evidence based medication.
* Actively support developing staff and students.
* Teach other members of the health care community to raise the profile of heart failure care.
