We are a unique organisation in the sense we are the only integrated provider in the Bath and North East Somerset, Swindon and Wiltshire system. We provide acute care for the Great Western Hospital, run adult community services across Swindon and manage primary care practices in Swindon, providing care for 30,000 patients. Our 5,600 staff have 1.2m patient contacts a year, with 13,000 operations carried out in our Theatres, around 4,000 babies born, and our emergency attendances are now well over 100,000 per year. Main duties of the job The successful candidate will be responsible for the delivery of high quality care to a defined patient group either within the acute hospital or community based setting. They are responsible for the assessment, planning, delivery and evaluation of evidence based individualised patient care within the boundaries of their role. The post holder will contribute to the continuing development of standards of care including developments of nursing standards and monitoring of outcomes. The post holder will be expected to continue with their own professional development.
