We provide high quality rehabilitation and reablement in the community provided alongside step-down intermediate care in the community bedded settings to adults (aged 18 years or over) who need support after discharge from acute inpatient settings and virtual wards to help them rehabilitate, re-able and recover in Cornwall.
The post holder will treat patients with complex and varied pathologies and presentations, including a wide variety of conditions e.g., neurological, orthopaedic, respiratory, frailty, and falls. Every contact, treatment plan, and working environment are different bringing variety and challenge to each working day.
Main duties of the job
1. To work as a lead clinical specialist Occupational Therapist to assess and provide complex specialist interventions, treatment and advice providing episodes of care for individuals eligible for Continuing Health Care (CHC) funding.
2. To provide a high standard of clinical service, performing advanced assessment of patients including those with highly complex presentations and from this formulating a diagnosis and treatment plan, carried out as an autonomous practitioner. Will have acquired specialist knowledge in specialist area.
3. To provide high-quality person-centred care which always considers people's safety, privacy and dignity using advanced clinical knowledge that is evidence based.
4. To collaborate with the Team Manager in planning, co-ordinating, delivering and evaluating the specialist service and area of specific clinical specialism to ensure a high standard of specialist service provision. Linking with the relevant Professional Lead as required.
5. To implement, monitor and evaluate Clinical Governance within specialist field, specifically looking at best practice and national guidelines, auditing, service evaluation and research.
6. To provide a leadership role that supports staff clinically, professionally and educationally in the specialist area.

Person Specification
Education /Qualifications / Experience
* Have an appropriate degree or equivalent knowledge and expertise.
* Relevant professional qualification and HCPC Registration.
* Masters level professional education supplemented by specialist training and development or equivalent experience.
* Significant post qualification experience as a senior Registered Practitioner.
* Working knowledge and experience of application of legislation and guidelines relevant to the role.
Skills and Aptitude
* Proven clinical leadership skills.
* Strong commitment to teaching and learning.
* Analytical skills in assessing both clinical and generic health and safety service risks.
* Proven IT skills with application to practice.
