Job summary
A position has arisen for a Healthcare Assistant to join our team.
We are looking for a motivated, enthusiastic, committed individual to join our friendly and forward-thinking supportive practice. The candidate will complement our established nursing team which currently consists of a nurse practitioner, three practice nurses one of which is community based and a full-time health care assistant.
Your role will involve some in-house work as well as some domiciliary and home visit work, you will therefore be expected to visit patients who are housebound in their own home.
We are a practice that is strongly focused on working together to serve our deprived community and help, where possible, address inequalities.
Due to the nature of the role, hours and salary are negotiable and dependent on experience. We expect the role to be between 30-35 hours per week.
Main duties of the job
Working as a part of our nursing team under the supervision of a nurse you will be competent to undertake in a variety of tasks including:
1. New patient health checks
2. Supporting the nurses with chronic disease reviews and disease management
3. BP monitoring & overseeing the BP Loan scheme
4. Urinalysis & preparation of specimens for investigation
5. Phlebotomy
6. Weight monitoring
7. Vaccine/cold chain storage, monitoring & recording including the rotation & disposal of vaccines & drugs within area of responsibility
8. Surgical equipment & vaccine re-stocking and stock rotation
9. Restocking of clinical areas & consulting rooms
10. Supporting the Nursing team & the GPs in the administration of seasonal vaccines
11. Administration of B12 injections
12. Dressings

About us
We are a seven partner GMS teaching practice in the North of Sheffield with a practice population of approximately 9,300 patients. Our team also includes 3 salaried GPs, 1 Nurse Practitioner, 4 Practice Nurses, 3 Health Care Assistants and a team of reception, secretarial and admin staff. We have a spacious, modern building.

Administration and professional responsibilities
31. Participate in the administrative and professional responsibilities of the practice team.
32. Ensure the clinical computer system is kept up to date with accurate details recorded.
33. Ensure appropriate items of service claims are made accurately, reporting any problems to the practice operations assistant.
34. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
35. Attend and participate in practice meetings as required.
36. To create and send recall letters for disease management.
37. To assist in seasonal and special projects as requested flu campaign.
Management of medical records
Ensure correspondence, reports, results etc are filed in correct records.
Management of appointment system
38. Ensure total familiarity with all appointment systems, including regular and incidental variations.
39. Book appointments and recalls, ensuring sufficient information is recorded to enable retrieval from the medical record.
40. Accurately and correctly record diary entries and recalls.
41. Monitor effectiveness of the system and report any problems or variations to the operations assistant.
