The role is based in Tamworth and is a permanent role.
Main Responsibilities
You will:
* support the Family Practitioner Leads to carry out the Local Authority's SEND duties in relation to co-ordinating Education Health & Care (EHC) assessments
* produce EHC Plans and carry out annual reviews
* work closely with our children and young people with special educational needs and disability and their families as well as a range of qualified professionals in education, health and social care
* develop collaborative and respectful relationships, promoting choice and independence and improving outcomes
* ensure that education, health, and care plans comply with legal, regulatory, ethical and social requirements in addition to the policies, standards and procedures
The Ideal Candidate
It's a fast-paced role so you'll need to be able to manage your own caseload independently and as part of the team.
You will also:
* need excellent written and verbal communication skills
* have knowledge of the SEND legislation plus, it's important that you can reflect on practice and improve as a result
