Job summary
We are seeking an experienced Practice Nurse to work in conjunction with our existing nursing team which comprises of Advanced Nurse Practitioners, Practice Nurses and Health Care Assistants (HCAs).Responsibilities will be for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient successful candidate will need to work across all three of our sites, must be well presented and have good IT skills. Previous GP Practice experienceessential, SystmOne clinical system experience essential. Salarydepending on experience.
Main duties of the job
1. To plan and deliver high quality integrated nursing care to the practice population
2. To promote health and well-being within the practice population
3. Running chronic disease management clinics
4. To undertake Cervical Cytology Screening
5. To implement vaccination programmes for children and adults
6. To administer travel medicine services (desirable)
7. Contraception and family planning
8. To work with clinical and non-clinical practice staff, other members of the wider healthcare team and NHSE/ICB in contributing to:
9. Deliver objectives set out in the NHS Action Plan Meeting the standards as set out by the Care Quality Commission (CQC)
10. Strongly contribute to the attainment of QOF targets
11. Deliverance of Enhanced Services

Job responsibilities
To provide and maintain an extremely high standard of nursing care forpatients across two GP surgeries and branch surgery as well as providing nursing assistance to thedoctors and other members of the primary healthcare team. The duties willinclude all tasks normally undertaken and in addition any roles agreed betweenthe nurse and the doctors as appropriate, having regard to current training.Take responsibility for chronic disease registers and work with admin staff tomaintain robust call and recall systems for chronic disease, cervical cytology,immunisation and other populations based health promotion and diseaseprevention care programmes, escalating where necessary patients who needintervention from a GP to ensure a review as appropriate, constructing carepathways where patients receive appropriate investigations and reviews by anappropriate clinician at the appropriate time interval.
