Job Description
We are seeking a highly motivated and focused Clinical Pharmacist - IP to work full-time within The Park Practice, part of Penge PCN.
The successful candidate will be responsible for the smooth running of day-to-day activities in general practice, providing pharmaceutical care and advice to patients while adhering to safety guidelines and best practice.
The clinical pharmacist-IP will work closely with a team of pharmacists and pharmacy technicians, supporting and working alongside them to deliver high-quality patient care.
Key Responsibilities:
* Resolving medication-related queries and improving patient outcomes
* Conducting polypharmacy medication reviews and SMRs, face-to-face medication reviews, and medicine reconciliation and review post-discharge
* Helping to utilise skill mix, ensure better access to healthcare, and support practice staff with prescription and medication queries
* Delivering clinical leadership on medicines optimisation and quality improvement, ensuring appropriate and efficient use of medication
* Educating patients on medication and disease prevention
Requirements:
* A minimum of 12 months' experience working within general practice/primary care
* A relevant degree/qualification and GPHC registration as a Clinical Pharmacist
* Independent prescribing qualification and experience with EMIS or System 1
* Excellent communication and interpersonal skills
