Occupational Therapist - Fall Prevention
The Optalis Short-Term Support and Rehabilitation Team (STS&R) provides reablement care and support to people who have recently been discharged from hospital. Reablement is a short-term, goal-oriented support service aimed at helping people regain their independence after an illness, injury, or hospitalisation. Instead of doing tasks for someone, reablement professionals work with individuals to build or rebuild skills so they can manage daily activities on their own, such as dressing, cooking, or moving around safely. The focus is on empowering people to maintain as much independence as possible, improving their confidence and quality of life.
STS&R is a multi-disciplinary team consisting of Occupational Therapists, Physiotherapists, and Rehabilitation Care Assistants providing support to customers across the Royal Borough of Maidenhead and Windsor.
Main duties of the job
We are looking for a qualified Occupational Therapist who is registered with HCPC and has experience of rehabilitation and supporting people in the community.
This role sits under our Keep Safe, Stay Well service in the STS&R team. KSSW is a service designed to provide early help so that people stay as well and independent as possible, and has a particular focus on falls prevention. The service actively helps people over the age of 18 who may struggle to live independently, has had a fall or may be at risk of falling, as well as providing information about managing health and ageing well.
Duties will include assessing customers and living conditions, providing equipment, guidance and making suitable adjustments to allow them to continue living as independently as possible. You'll work alongside our team of OTs and Physiotherapists to help agree a person-centred support plan.
The role is based out of Maidenhead Town Hall and will include working in the community, visiting customers at various locations across the borough. It is essential that you are able to drive and have your own vehicle.
Job responsibilities
* To provide falls prevention education, information and advice to individuals, family/carers and groups.
* To participate in the Keep Safe Stay Well triage process.
* To provide outcome focused assessments and interventions.
* To assess and prescribe equipment and identify and progress the need for adaptations to Service Users accommodation as appropriate.
* To manage a caseload including complex assessments.
* To ensure appropriate safeguarding processes are in place for the most vulnerable residents.
* To engage in service promotion and development e.g., public/professional events, presentations, and training.
* To provide support and/or supervision to therapy assistants within the team.
Person Specification
Qualifications
* Qualification in Occupational Therapy.
* Registered with HCPC.
* Good knowledge of the needs of vulnerable adults and their carers.
* Knowledge of the principles and practice of risk assessment.
* An awareness of the function and operation of Eligibility Criteria within Adult Social Care.
* Sound understanding of Self Directed Support.
* Full UK Driving Licence.
Experience
* Ability to demonstrate a high level of understanding of Falls prevention and all relevant legislation e.g. NICE guidance Falls in older people.
* Experience of working with Adults in the community.
* Good time management skills, able to prioritise workload and work under pressure.
* Ability to use initiative, take control in a crisis situation and remain calm.
* Previous experience of working in a Falls Prevention environment.
