Spencer Clarke Group are seeking a Case Manager for a Local Authority Client based in North London.
In this role, you will work collaboratively with patients, carers, and multidisciplinary teams to deliver holistic, person-centred care for older adults, improving their quality of life and reducing unplanned hospital admissions.
About the role:
You will be tasked with delivering key strategic objectives:
1. Conduct assessments, develop personalised care plans, and oversee seamless coordination of health and social care services for older adults with complex needs.
2. Serve as the main contact for patients and carers, providing guidance, monitoring progress, and ensuring care plans are regularly reviewed and updated.
3. Partner with multidisciplinary teams, including GPs, social care, and voluntary sector professionals, to deliver integrated, person-centred care.
4. Manage a caseload, maintain accurate records, and contribute to the continuous improvement of care processes and outcomes.
About you:
The successful candidate will have the following skills/experience:
1. Experience managing caseloads and coordinating care for individuals with complex health and social care needs.
2. Experience conducting comprehensive assessments of medical, emotional, and social needs.
3. Proven ability to work effectively with multidisciplinary teams, including GPs, social care, mental health professionals, and voluntary sector partners.
4. Experience in health or social care settings, including knowledge of relevant policies like the Care Act and Mental Capacity Act.
5. Ability to handle complex cases and make informed decisions under pressure.
What's on offer:
Salary: £20+ph
Contract type: 3-6 months minimum, with a high likelihood of extension.
Hours: Monday to Friday, 36 hours per week.
