We are excited to be recruiting a temporary 9 month contract for a Band 4 practitioner, to work in our Mental Health Access Service at St Mary's Hospital. Working in Portsmouth offers a diverse inner-city client base. We want you to come and join us helping our service users connect to the most suitable service for their mental health needs.
We are expanding our team who provide Portsmouth residents with quick access to a range of services for expert assessment and interventions. This includes a telephone line that residents can access; our team of call handlers will ensure we connect the service user to a suitable pathway linking in with a variety of other agencies.

Main duties of the job
1. To demonstrate an empathic understanding of mental health problems and issues.
2. To actively listen and autonomously help the service user to initiate a care plan, providing them with recommendations of services or linking them in with the appropriate teams.
3. To liaise with other agencies including Mental Health Teams with regard to the transfer of care to the appropriate agency.
4. To identify and provide the necessary support/advice to patients on practical issues relating to living skills such as nutritional management, self-care, social skills, benefits, housing, education, training, and occupational activities.
5. To maintain effective relationships with members of the Access team and the wider community.
6. Maintain effective relationships with the service user, relatives, friends, and other involved helpers.

Person Specification
Qualifications
* NVQ 3 or Health Related Qualification or equivalent experience
Experience
* Experience with working with people and patients, carers.
* Experience of working within a multidisciplinary team.
* To have a working knowledge of Risk Assessment and management tools currently in use within acute mental health settings.
