Our Directorate of Planned and Proactive Care: Community services is seeking a Band 5 RGN to support the Community Cardiorespiratory teams in North Nottinghamshire; Bassetlaw. This is a really exciting opportunity to be part of a specialised community team.
To be pro-active in delivering anticipatory and responsive care, promoting independence, health lifestyle, and reduction in health inequalities to improve the overall quality of life in the local population for cardio-respiratory patients.
To offer a comprehensive nursing / therapy service that is a multi-disciplinary and multi-agency approach to maximise patient function, well-being, and self-care planning.
To work collaboratively with the respiratory physiotherapists in the planning and delivery of pulmonary rehabilitation programmes.
To support ongoing service development of Adult Community Health Services within Bassetlaw to achieve a model of wrap-around care that is responsive to the needs of the patients / service users.
Main duties of the job
As the Band 5 Cardio-respiratory nurse you will:
* Support with specialist clinical assessment of people with Heart failure and COPD/ Respiratory conditions.
* Develop skills and competencies in health assessment, medicines management, and case management to support patients with Heart Failure COPD / respiratory conditions.
* Work collaboratively and integrate with primary, secondary, social care, the voluntary sector and with patients to establish a whole systems approach to managing health and social care for the designated patient group.
* Provide support to both students and other professionals.
* Work within Pulmonary Rehabilitation and undertake pre/post assessment and individualised Exercise programs for patients.
* Support and educate patients and carers about their Heart failure and COPD/Respiratory condition so they are equipped as far as possible with the knowledge and skills to manage their condition to their maximum potential.

Detailed job description and main responsibilities
The search has begun for a cardio-respiratory nurse within the Planned and Proactive Care Directorate; Community Cardiorespiratory, North Nottinghamshire, Bassetlaw and surrounding locality.
We are looking to recruit an enthusiastic, creative and driven band 5 nurse to support both the pulmonary rehabilitation and heart failure service.
This is an exciting opportunity to join existing teams and help shape services as we move forwards in our planned and proactive care directorate.

You must be an NMC Registered clinician with ideally community and Cardiorespiratory experience with evidence of continuing professional development and knowledge of working with people with complex physical health problems, to inform and support the clinical care pathways; preferably within a community setting although clinical skills and experience are transferable.
You will need to be highly motivated, have positive effective interpersonal skills and a commitment to promote recovery and rehabilitation-oriented practice with the aim of preventing unnecessary and reducing hospital admissions in a variety of ways.
A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
