Lancashire County Council (LCC) is seeking a skilled and experienced Grade 8 Social Worker to join our Adult Services Team. This role offers the opportunity to make a meaningful impact in both community and hospital settings by providing strength-based, outcome-focused assessments to adults and their carers.
Key Responsibilities
1. Deliver a professional social work service, applying a strong understanding of statutory duties in line with national legislation and organisational policies.
2. Promote independence, recovery, and safeguarding through strengths-based, outcome-focused interventions.
3. Collaborate with other professionals and agencies to address complex needs and achieve positive outcomes for individuals and their carers.
4. Conduct high-quality assessments and develop tailored support plans in consultation with service users and carers.
5. Prepare and present written reports in various formats as required.
6. Facilitate safe and timely hospital discharges, supporting individuals' choices and independence.
7. Promote innovative use of personal budgets to meet needs effectively.
8. Maintain clear and effective communication with individuals, carers, professionals, and partner agencies.
9. Manage and prioritize a caseload efficiently.
10. Maintain accurate, up-to-date records using Liquid Logic and other relevant systems.
11. Contribute to team effectiveness, service improvement, and assist in mentoring less experienced colleagues or students.
12. Commit to ongoing professional development, including mandatory training.
13. Maintain emotional resilience and professional wellbeing while building confidence in practice.
14. Participate in a flexible duty system to respond to urgent situations.
What We're Looking For
1. Essential qualifications: Qualified Social Worker with valid registration.
2. Experience: Demonstrated expertise in adult social care, particularly in hospital or community settings.
3. Skills: Strong communication, case management, and collaborative working skills.
4. Knowledge: A thorough understanding of relevant legislation, safeguarding practices, and person-centered approaches.
