We are looking for an enthusiastic, confident experienced Renal nurse to join our team as a band 6 home therapies nurse. You should have experience in both peritoneal and haemodialysis and should be able to draw on knowledge and skills in coordinating the care of patients who choose to have dialysis in their home environment.
You will be required to provide excellent nursing care, utilizing the specialist knowledge to teach and educate patients undergoing Peritoneal Dialysis. You should be a confident educator for both patients and staff and be able to have a flexible approach in meeting the needs of patients. You will work collaboratively with the wider renal multi-disciplinary team and support the renal ward with staff training and clinical expertise.
The successful applicant will demonstrate excellent communication skills. You will have outstanding organisational skills be able to work autonomously as well as being part of the wider Home Therapies Team. The role includes travelling to visit patients in their homes as part of their care and follow up. Access to a car and the possession of a UK driving license is essential to this post.
Responsibilities include:
1. To be responsible for the care of a caseload of patients requiring specialist nursing care within the hospital and home setting.
2. To teach patients how to carry out their own dialysis treatment at home and to provide specialist nursing care and support to patients and their carers.
3. Educate patients and provide counselling on different treatment options for patients with Advanced Kidney diseases.
4. To work collaboratively and coordinate the development of the home dialysis unit making best use of available resources.
5. To provide an effective and responsive service to patients and carers within the trust and in their homes.
6. To provide practice development support to renal nurses.
7. Participate in the procurement of dialysis consumables and the home delivery services as appropriate.
