A) To lead, oversee and drive forward effective and efficient patient flow,ensuring continuous improvement across pathways 1-3 at St Richards Hospital. b) Provide a senior operational interface with other senior staff within the health and social care system. c) To lead a site -based Transfer of Care Hub with the authority and capability to make decisions with multiple organisations to maximise the quality, efficiency and effectiveness of all hospital discharges/transfers. d) To produce TOCH updates for the named individual site, being the first line of escalation to system challenges and pressures including producingappropriate performance reports for the system. e) Lead on the named site operational and performance meetings including daily calls /huddles and being the identified SCFT lead for site MADE events (for all pathways). f) To work closely with matrons and managers of in-patient Intermediate Care Units, Urgent Community Response teams and Virtual Ward teams to ensure optimal use of capacity ensuring patients are transferred to the most appropriate SCFT community service. This includes having responsibility for mapping patients into available SCFT beds and (where appropriate) Brighton & Hove contracted beds. g) To contribute to the development of the Trusts operational policies relating to patient flow and discharge. h) To Interpret and translate into operational practice national and local best practice guidance with respect to maximising patient flow. i) To work collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised. j) To support ward staff and patients and their families to agreedischarge/transfer plans providing practical solutions to prevent unnecessary delays. k) Provide key leadership and effective line management of designated staff within the Capacity and Flow team. l) To be responsible for the financial management of an identified Capacity and Flow team budget
