Waythrough launched in October 2024 following the merger of Humankind and Richmond Fellowship. Our vision is to break down the barriers that stop people getting the support they need to live a life they value. We tackle poverty and disadvantage in communities, through mental health, drug and alcohol, housing and related support.
We have almost 200 services around England – and nearly 3,500 amazing staff and volunteers who run them. Every year our services support around 125,000 people.
Bringing together two big organisations is not a small task and it’ll take a while to fully integrate our systems, processes and approaches. That’s why you will notice the jobs here are still branded ‘Humankind’. Next year we’ll be fully integrating our recruitment systems but for now they are separate.
Waythrough's Durham Acute Hospital discharge service works in partnership with Homegroup. Homegroup work in Hospitals with the service users and Waythrough supports upon discharge and in the community.
About the role
* Manage a caseload of service users, completing and reviewing high quality and comprehensive assessments, risk assessments and support plans, including liaising with family members, significant others and professionals / agencies as appropriate.
* Implement, develop and assist in the management of services delivered by the team.
* Manage, mentor, coach and ensure the development of staff reporting to you.
* To be responsible for supervisions for staff and support and guide through safeguarding concerns.
Waythrough Hospital discharge service aims to provide support to maintain or improve health and wellbeing, with rapid, timely support to manage concerns which may arise following discharge from hospital. The services complete a full assessment of their needs and provides a holistic and person-centred plan to help support them while out in the community. The support provided includes, maximising income and managing their tenancies, finding suitable accommodation if current accommodation is not meeting their needs, ensuring they have access to appropriate health services, signposting to specialist services within the County, assistance to attend appointments when required, build social networks in the community and access grant funding for items and services required.
By providing this support, the service in turn reduces the need for patients to be readmitted to hospital after discharge; by ensuring they have a support network around them in the community and the accommodation is suitable to their needs.
