Please refer to the supporting documents section to retrieve the JD detailing the core responsibilities of this role. Your job is to work directly with key stakeholders to help us harmonise ways of working and improve working practices to enhance patient and staff satisfaction. You will be the key link to the people whose care you are supporting, operating as a go-to person to ensure that their care is seamless, that they are active decision-makers in their own care, and that everyone involved is working together. Care coordinators provide extra time, capacity, and expertise to support patients in preparing for or following up on clinical conversations they have with primary care professionals.
You will love this job if you are able to work sensitively with patients, their families, and carers, are able to work as part of a healthcare team to seek feedback, continually improve the service, and contribute to business planning. If you feel this is the ideal opportunity for you and you want to rise to the challenge, we welcome your application to join our growing team of like-minded people.
