Contribute towards the provision of an effective Personal Advisor service for children and young people in care, preparing to leave care or who have left care. Discharge the personal advisor role and take responsibility for an individual case load and pathway planning. Key Responsibilities: Achieve the high aspirations and ambitions that they have to ensure Care Leavers are supported to reach their full potential. Ensuring timely and meaningful visits and pathway planning for young people to evidence their part in planning for their future. Build meaningful, trusting relationships where Care Leavers feel safe, valued and respected so they have a successful adult life. Work together with care experienced young people to continually shape and improve the Leaving Care offer for Wakefield care leavers. Undertake direct work with non complex or post 18 individuals and take the lead role in reviewing young people’s Pathway Plans and Risk Assessments. Carry out actions as outlined in the Pathway Plan and be involved in the preparation, coordination, and review of the Pathway Plan in accordance with required standards and timescales. Provide a comprehensive and personal support to all assigned cases and carry out practical tasks relating to all aspects of the Pathway Plan Delegate relevant aspects of the work accordingly to the specialist roles within the service and external agencies as appropriate Qualifications: NVQ Level 3 or equivalent Knowledge: Knowledge and understanding of the difficulties young people face in making the transition from adolescence to adulthood. Knowledge and understanding of the Children (Leaving Care) Act 2000 and other related legislation. Knowledge of Safeguarding issues. Experience: Recent relevant experience, paid or unpaid, of working with or supporting this user group or other such vulnerable young people. Experience of team working. Experience of interagency working. Skills & Abilities: Good standard of ICT skills. Listening skills Keyboarding skills are required to ensure accurate and appropriate information is produced and provided to service users, customers and clients as required. Ability to engage and work positively with children and young people some of whom may have complex needs. Ability to carry a caseload and plan and review work effectively.
