Hartlepool Borough Council is seeking a dedicated and passionate Children's Social Worker to join our Disabilities Team. We are committed to providing high-quality support and advocacy for children with disabilities and their families, empowering them to achieve their full potential.
About the Role:
You will be part of a team dedicated to:
* Providing tailored support and interventions to children with disabilities and their families, addressing their needs and promoting their well-being.
* Working collaboratively with children, their families, schools, health professionals, and other agencies to create individualised care plans.
* Advocating for the needs of children with disabilities, ensuring their rights and access to appropriate services and resources.
* Promoting inclusion and supporting children to participate fully in their communities.
Responsibilities:
* Conduct comprehensive assessments of children with disabilities and their families, identifying their needs, strengths, and aspirations.
* Develop and implement individualised care plans, tailored to the specific needs of each child and family.
* Provide direct support to children and families through one-to-one sessions, group work, and community outreach.
* Facilitate access to specialist services and resources, such as education, healthcare, and social inclusion opportunities.
* Work collaboratively with other professionals and agencies to ensure children receive the best possible care and support.
* Monitor progress, evaluate interventions, and make adjustments as needed.
* Advocate for the needs of children with disabilities, ensuring their rights and well-being are protected.
* Maintain accurate records and documentation, adhering to relevant legislation and guidelines.
* Participate in regular supervision, training, and professional development opportunities.
Key Requirements:
* Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
* Social Care England registration.
* Updated DBS check.
* Experience working with children with disabilities and their families.
* Knowledge of relevant legislation and guidance related to supporting children with disabilities.
* Excellent communication, interpersonal, and advocacy skills.
* Ability to build rapport with children, families, and other professionals.
* Commitment to supporting children with disabilities to achieve their full potential and live fulfilling lives.
