Job summary A rare and exciting opportunity has arisen for an Assistant Practitioner to join the existing Intensive Home Treatment Team (IHTT). We are looking for a highly motivated and enthusiastic person with experience of working with older adults with mental health problems.This is a multi-disciplinary service working in the Gedling, and Hucknall localities. The service is for older people with mental health needs and is delivered in people's own homes/care homes. The service works along side the Community Mental Health Team (CMHT) and makes every effort to prevent unnecessary admission to hospital or residential care. You will contribute to assessments and interventions, undertaking nursing and therapy assessment and intervention under the supervision of a registered clinician. The service is available 7 Days per week during the hours of 08:00 - 20:00. You will frequently work without direct supervision in the community and be required to work in a flexible and holistic way in order to respond to this client group's needs. Main duties of the job This role requires working across differing professions including nursing, occupational therapy, physiotherapy and social work. You will undertake assessments and interventions which are competency based, working in a flexible and holistic way to meet the needs of this patient groups. You will take an active role in the development and supervision of community support workers, as well as contributing to the development of the service. Responsibilities will also include supporting with the community support workers' rotas and allocating daily work.
