Responsibilities
Conduct comprehensive assessments of clients' needs and circumstances.
Develop and implement personalized care plans in collaboration with clients and their families.
Provide information and advice on access to services and resources available in the community.
Monitor and review clients' progress and adapt care plans as necessary.
Advocate for clients' rights and ensure their voice is heard in all aspects of care delivery.
Collaborate with healthcare professionals, community organizations, and local authorities to coordinate care services.
Maintain accurate and up-to-date records of assessments, care plans, and client interactions.
Requirements
A relevant qualification in social work or equivalent experience in a similar role.
Registration with the appropriate professional body (e.g., Social Work England).
Demonstrated ability to work effectively with vulnerable adults and their families.
Strong interpersonal and communication skills to engage with clients and stakeholders.
Experience in conducting assessments and developing care plans is preferred.
Knowledge of relevant legislation and best practices in social care.
Ability to work autonomously and as part of a multidisciplinary team.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £23.86
Location: Westminster, South West London
