We are recruiting Care Coordinators to join our team!
This role will be working in the North West Primary Care Network.
This is an exciting opportunity in which you will gain experience and make a real impact. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within the Primary Care Networks.
The ideal candidate should either be educated to NVQ Level 3 or have equivalent knowledge and experience of office procedures; including a wide range of administrative systems and software. Previous experience of Primary Care or the NHS would be an advantage, however is not essential.
The successful candidate will need to be flexible in their approach to work as the role will be working primarily across the Primary Care Networks, within various practices in Doncaster to undertake duties including:
1. Help and support the multi-disciplinary team (MDT) meetings and patient case management.
2. Support the MDTs to deliver effective and co-ordinated care for vulnerable and frail adults; including those at high risk of hospital admission.
3. Help people manage needs, e.g. answering queries, managing appointments.
4. Assist people to self-manage their health and wellbeing.
5. Support patients to utilise decision aids in preparation for shared decision-making conversations.
