We are searching for a qualified and experienced mental health practitioner, with significant post-registration experience, who is passionate about improving service user and family experience, reducing out of area placement and commissioning high quality recovery-focused care and treatment for individuals with a learning disability and/or autism, to enhance our clinical team.
The role is within the Individual Patient Placement (IPP) Directorate based on the Langdon Hospital site in Dawlish. The IPP directorate commissions high quality services that are as close to home and facilitates repatriation back to Devon as soon as clinically appropriate to the least restrictive environment.
Applicants must be natural problem-solvers, forward-thinking, innovative, compassionate, creative, and be willing to travel and stay all over England and Wales. The role is to work with people who use services and providers of their care to ensure a timely and smooth transition from hospital.
This post will be offered at 0.6 wte (22.5 hours per week)
Main duties of the job
The Post holder will work to expedite discharge for those from Devon in out of area hospitals, or in area independent sector high dependency Inpatient Rehabilitation units in area, prioritising those who have been in hospital for two years and over. The post holder will liaise with key stakeholders and troubleshoot blockages to discharge, working closely with health and social care colleagues. This will include supporting assertive projected mapping of individuals discharge plans of pre-determined cohort of patients.
The Post holder will participate in Care (Education) & Treatment Reviews (CTEs) and conduct eight-week safe and wellbeing checks/commissioner oversight visits. They will also contribute to the performance and quality monitoring process for providers, alongside Case Managers/FIND to improve quality and outcomes for patients.
Reporting to the Learning Disability and Autism Partnership Group and to the monthly tracker meetings as required.

Communication and Working Relationship Skills
* To work under the direction of the IPP Specialist Placement and Review Manager and support Devon ICB Learning Disabilities & Autism Partnership (LDAP) Team, providing expertise, knowledge, and leadership in discharge planning with inpatient teams for patients currently in hospital under the Learning Disability and Autism Programme (LDAP). The post holder is required to work autonomously without direct supervision, using their own initiative to resolve, manage, and escalate difficult and unexpected situations as they occur.
* To effectively communicate and negotiate around a wide range of sensitive and complex issues with a wide range of stakeholders.
* To represent Devon ICB/DPT in facilitation meetings to ensure two-way communication between in and out of area providers to resolve any barriers to transfer.
* To act in a consultative capacity to all services across Devon and other agencies regarding all aspects of discharge planning.
* Provide leadership and resolution where there is conflict regarding care and support of service users.
* Contribute to the team's engagement with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies.
* Impart complex and sensitive information to patients and families/carers regarding assessment, care planning, treatment and review, ensuring the provision of consistent well-coordinated care.

Analytical and Judgemental Skills
* To have senior oversight and influence in ensuring that people who have needs related to learning disabilities and autistic spectrum disorder and require a discharge plan with a funded element, receive the right package of care, at the right time in their own home or an appropriate setting.
* To participate in Care (Education) & Treatment Reviews (CTRs) and conduct eight-week safe & wellbeing checks / commissioner oversight visits.
* Be influential in and manage a process which ensures that flow in mental health inpatient settings is not prevented by delayed transfers of care.
* Provide expertise, knowledge, and leadership in support of care coordinators, lead professionals and MDTs.
* To undertake only those tasks which you are instructed and trained to do and are competent to fulfil.
* To interpret and work within the legal framework of the Mental Health Act 1983 and the Care Act 2014.
