We are currently looking for an enthusiastic, highly motivated Paediatric Diabetes Patient Navigator and Multi-Disciplinary Team (MDT) Coordinator to join our Paediatric Diabetes Team. This is an exciting opportunity to be part of an established service that supports the Trust's Paediatric Diabetes Team. We use information systems including clinical systems such as Lorenzo.
Main duties of the job
The main duties include working with the CYP Diabetes Team, providing support to the Clinical Nurse Specialists, to ensure timely care and optimal waiting list management for all patients & their parents/carers referred to the CYP Specialist Diabetes Team.
Applicants must possess a good standard of education and have experience in working within the NHS in an administrative role. The post holder must be able to evidence:
* Ability to work within a team framework
* Good organisational and administration skills
* Excellent interpersonal and communication skills
* Ability to maintain confidentiality of sensitive information at all times
* Experience of working in the NHS, ideally with knowledge of local and national KPIs

Person Specification
Education, Knowledge & Skills
* Educated to GCSE maths and English standard or equivalent experience
* Knowledge and use of medical terminology
* Experience of undertaking a range of administrative duties including diary management, telephone queries & excellent keyboard skills
* Equivalent of NVQ Level 3 in Business Administration
Experience
* Experience in using hospital IT systems such as Lorenzo.
* Experience of using Microsoft systems
* Working with spreadsheets and databases
Skills, Ability and Knowledge
* Evidence of good clear communication skills
* Proficiency in English language
* Awareness of quality management systems
* Knowledge of national performance targets
Personal Qualities
* Ability to work under minimal supervision
* Clear understanding of patient confidentiality
* Commitment to team working
