Job summary
We are looking for a highly experienced and forward thinking individual to join and operationally lead the existing dynamic Home 1st Planned Care Team. You will be joining us at an exciting time as we continue to develop our partnership working with the local acute trust, social care and VCSE colleagues. You will be working alongside our partners to design, implement and deliver the therapy pathways for patients in Milton Keynes.
You will also be responsible for continuing our work to embed the Community Rehab Strategy to ensure equitable access to services for all patients.

Main duties of the job
As a Directorate, MKCHS has eight Service Lines, one of which is Home First Panned Care.
You will be responsible for the operational delivery of Home First Planned Care which comprises of a number of services and includes:
1. Early Stroke Rehab Team
2. Home First Therapies
3. Neuro Clinical Specialist Team
4. Neuro Rehab Service
5. Pulmonary Rehab
6. Staying Steady
You will ensure there is robust and safe clinical, and financial management of the Service Line and work with the Service Directors to ensure that the strategic direction, as defined within the service, is effectively implemented at an operational level.
You will be responsible for the continued development of the service and the link in with key stakeholders to ensure a system approach is taken to meet the needs of the local population.
You will also head up a range of service development initiatives directly as well as working with and through your teams, colleagues, and peers to deliver service improvement projects.
You will be required to work closely with all of the senior management team. In particular you will work closely with the Manager for Home 1st Unplanned Care to deliver the admission avoidance pathways and discharge pathways in line with the local and national guidance.
