Forum Family Practice are looking to recruit a friendly, motivated, enthusiastic and experienced care navigator/data administrator to join our team.
You will play a crucial role in helping our patients both face to face and over the phone with their health care needs. The post holder will ideally have previous GP practice or NHS experience with knowledge of Systmone, however this is not essential.
If you enjoy a variety in your work, have IT skills along with excellent customer service then this is a perfect job for you.
We are looking for a minimum of 28 hours per week.

* To support the practice clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the practices multidisciplinary team
* Process patient requests for appointments.
* Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately.
* Initiate contact with and respond to requests from patients, team members and external agencies.

You will join a friendly clinical team, consisting of 5 Partner GPs, 1 Salaried GP, 2 Advanced Nurse Practitioners (ANPs), 2 Practice Nurses, and 4 Healthcare Assistants. Through our Primary Care Network (PCN), we also have access to additional resources, including 2 Link Workers, 3 Pharmacists, 2 Mental Health Practitioners, and a visiting Paramedic. Our medication technician team supports prescription queries and medication reviews, enhancing patient safety and accessibility. The non-clinical team, who will support and assist you, includes a managing partner, an office manager, and a dedicated 12-person administrative support team.
Job responsibilities
1. Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition.
2. Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately.
3. Process patient requests for appointments.
4. Initiate contact with and respond to requests from patients, team members and external agencies.
5. Data entry of new and temporary registrations and relevant patient information as required.
6. Input data into the patients healthcare records as necessary.
7. Manage all queries as necessary in an efficient manner.
8. Maintain a clean, tidy, effective working area at all times.
9. Support all clinical staff with general tasks as requested.
10. Undertake all mandatory training and induction programs.
11. Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.
12. Contribute to public health campaigns (e.g., flu clinics) through advice or direct care.
Secondary responsibilities
1. Action incoming emails when necessary.
2. Scan patient-related documentation and attach scanned documents to patients healthcare records.
3. Complete opening and closing procedures in accordance with the duty rota.
