We are looking for a practice based Clinical Pharmacist responsiblefor support to the practice with medication reviews, queries and prescribingsafety.
The new person will also be responsible forsupporting the partners in implementing effective medicine management withinthe practice. Supporting the pharmacyteam to identify areas for improvement and then initiate and manage the changes.
The successful candidate will also be responsible in leading the practice medicine monitoring reviews and supporting our pharmacy technician and prescribing clerk.
Main duties of the job
The post holder will work within their clinical competencies as part of amulti-disciplinary team to provide expertise in all aspects of medicinesoptimisation within the practice:
* Lead and oversee the high riskmedication monitoring with the practice admin and the in-house pharmacyteam.
* Provide a point of contact for thepractice for all medicines-related queries for healthcare professionals aswell as patients.
* Patient facing clinics are notcurrently required but if there are areas of specialty that could providebenefits to the practice then the surgery would be open to suggestions.
* Follow legal, ethical, professionaland organisational policies/procedures and codes of conduct.
* Oversee the MHRA alerts/CAS alerts,to make sure these are audited, actioned and logged on a spreadsheet.

* Reconciliation of medicines fromoutpatient and discharge letters including liaison with hospital,community and primary care colleagues to ensure correct medicines arecontinued following transfer of care.
* To work with the pharmacy team andsupport GPs and other practice staff to deliver on Medicines Optimisationagenda (Prescribing Quality Scheme), QOF and locally commissioned enhancedservices.
* To encourage, support and implementadherence to local formulary guidelines and cost-effective prescribing.
* Reviewing, issuing and convertingacute medicine requests into repeat medicines, where appropriate.
* Supporting Community Pharmacists inensuring NOMAD or batch prescriptions are up to date including confirmingany stopped or initiated medications with the patient and communitypharmacy team.
* Implementing and monitoring apractices adherence to a repeat prescription policy.
* Supporting the Practice PharmacyTechnician (or prescription clerks) with the management of the repeatprescription processes and other medicines management related duties.
* Contributing to repeat prescribingsafety i.e. prompting patients for reviews, blood tests, monitoring etc.
* Working with the multidisciplinaryteam to improve medicines management and safety within the practiceincluding updating, writing and ensuring compliance with practicepolicies.
* Dealing with patients medicine andprescription related queries over the phone, in clinic, via letter/emailor the practice website (this may include advising and prescribing forminor ailments or clinical areas within competence, acute prescriptionrequests, carrying out medication reviews, lead on medication monitoring, providingmedication advice, solving technical prescription problems such as supplyproblems, NOMAD changes etc.)
* Signposting patients to appropriateservices and other healthcare professionals (e.g. community pharmacists).
* Working closely with GPs to providemedicine related updates and helping with problem solving.

* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council

* Previous experience in hospital, general practice or CCG
* Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice
