You will play a key role in optimising patient outcomes through expert clinical practice, leadership, and education in medicines management. As an ACP, you will work within the Four Pillars of Advanced Practice to provide exceptional care in both face-to-face and remote settings, collaborating with a multi-disciplinary team and Integrated Care Board (ICB). You will help manage patients with long-term conditions, co-morbidities, and poly-pharmacy, driving improvements in clinical effectiveness and safety. Your contributions will directly impact the quality of care we provide to our community. This role requires a blend of clinical expertise, critical thinking, and a proactive approach to patient care. Key Responsibilities: Clinical Practice: Conduct comprehensive patient assessments, including history taking and physical examinations. Develop, implement, and evaluate individualised care plans, with a primary focus on medicines management. Prescribe and manage medications in line with clinical guidelines and best practices. Monitor and evaluate the therapeutic effects of medications, adjusting treatment plans as needed. Provide expert advice on the safe and effective use of medicines to patients, families, and healthcare professionals. Medicines Management: Lead and coordinate initiatives to optimise the use of medicines, minimise medication errors, and improve patient safety. Develop and implement policies and procedures related to medicines management. Conduct clinical audits and participate in quality improvement projects to enhance practices. Collaborate with pharmacy, nursing, and medical staff to deliver comprehensive medication management. Education and Training: Educate healthcare staff on medicines management and pharmacotherapy to foster best practices. Mentor and support junior staff and students, contributing to a culture of continuous learning and development. Leadership and Collaboration: Participate in multidisciplinary team meetings and contribute to the development of clinical guidelines and pathways. Represent the organisation at local, regional, and national forums related to medicines management. Act as a resource and advocate for patients, families, and staff on medication-related issues. What We’re Looking For: A registered healthcare professional (Nurse, Pharmacist, or Allied Health Professional) with relevant post-registration qualifications. Advanced Clinical Practitioner qualification (Master's level or equivalent). Independent prescribing qualification. Significant experience in a clinical role, particularly in medicines management. A solid understanding of pharmacology, therapeutics, and clinical guidelines. Excellent communication, leadership, and interpersonal skills.
