The aim of Adult Community Services is to provide a first-class healthcare service across the in-patient and community services. We provide services to a broad multicultural community throughout Hackney and the City. We have Deputy District Nurse Team Leaders vacancies within the Adult Community Services Department to provide nursing care to allocated patients within their homes. There are 2 roles: one is based at John Scott Health Centre, and the other one is based at Fountayne Rd Health Centre.
Main Duties of the Job:
The post holder will be responsible for the planning, implementation, and evaluation of nursing care needs and the delivery of high-quality and up-to-date care to allocated patients in their own homes and/or a variety of settings. The post holder will work within a team for a defined number of patients and will work under the supervision of the Clinical Operations Manager. You will be responsible and accountable for the management of a caseload in the absence of the Clinical Operations Manager/District Nurse. You will also work closely with the aligned GPs to your team and will be both the advocate and the key chain of communication for their care.

Person Specification
Education/Qualification Essential:
* Registered General Nurse (NMC Registered)
* Physical Assessment Course
* Mentorship and Preceptorship training
* Specialist Practitioner District Nursing Qualification
Desirable:
* Management training
Skills and Abilities Essential:
* Ability to manage junior staff members, delegating appropriately, and to manage workload effectively
* Ability to meet deadlines as required.
* Able to work unsupervised in the community setting.
* Able to work using your own initiative
* Ability to communicate effectively and appropriately in a wide range of settings
Experience Essential:
* Post registration experience within the community
* Experience of undertaking first contact/assessments in the community
Desirable:
* Experience in line management
