Duties may be varied from time to time under the direction of the Repeat Prescribing Team Leader / Operations Manager, dependent on current and evolving Practice workload and staffing levels. Process repeat prescriptions on a daily basis, ensuring compliance with the practice protocol. Perform regular searches on drugs requiring monitoring, hence minimising the risk of adverse effects. Ensure all current medications have relevant, current authorisations. Promote cost-effective prescribing at the practice by switching drugs to generic preparations when appropriate and encouraging the use of local primary care trust formulary medication choices. Deal with prescription queries from a range of healthcare professionals, including GPs, nurses, community pharmacists, nursing home staff, and reception/administration staff at the practice. Deal with medication changes from all hospital letters, removing old medication and adding new or making changes. Write a letter to the patient, including the new RHS and confirmation of changes. Add any relevant monitoring diaries. Working closely with the Prescribing Support Pharmacist. Perform searches and act upon drug alerts. Add new patient medication. Pass to registered GP for approval, highlighting any concerns and relevant diaries. Liaise with patients via phone call or letter.
