Acuity Care Group Limited | Bank Paediatric Nurse
We are seeking a passionate and driven Bank Paediatric Nurse to join our team. The primary purpose of the Nurse is to support complex care packages within the person's own home. We specialise in high acuity complex needs including tracheostomy and ventilator support. This is a field-based role involving regular travel across the Northwest region as well as working from home.
This service is delivered through teams of support workers who report to the Complex Care Manager (CCM). Accordingly, the CNS has direct responsibility for ensuring the capability and standards of service delivered by these teams.
Critical within this role and underpinning the delivery, is the need to operate both flexibly and reliably, whilst fiercely ensuring Bespoke's values around client-centred care are never compromised and that all regulatory compliance standards are diligently adhered to.
The role of the Paediatric Nurse is broken down into two main areas as follows:
* To support the safe transition and mobilisation of our complex packages from hospital to home.
* The ongoing maintenance of services for existing clients, ensuring ongoing high standards of care and safety are constantly achieved.
* Support with a mixture of day and night shifts.
Reporting directly to the Clinical Nurse Manager and working collaboratively with Complex Care Managers (CCMs), ICBs, other professional bodies and key team members in the organisation, this is an important role contributing to the successful performance of the business.
New client set up:
* Work collaboratively with the Mobilisation team and operational team to coordinate the set-up of a new client package, including undertaking the clinical assessment to define the clinical care requirements of the package.
* Provide the clinical training to the support workers, as required and work collaboratively with the CCM to schedule relevant shadow shifts to facilitate an efficient sign off process.
* Escalate any concerns around worker capability in a timely manner.
* Document worker competency sign offs in line with the agreed process and update on People Planner.
* Develop the care plan and produce all clinical documents required for the set up of the package on our electronic system PASS.
* Regularly liaise with the client and support workers to ascertain the running of the client's package and make necessary alterations for continuation of care.
* Set up the competency requirements for the client on People Planner.
* Liaise with the CCMs regularly to review care packages being delivered in accordance with the defined clinical standards and implement timely remediations if required.
* Monitor and audit the completion of clinical paperwork (e.g. MAR charts) ensuring any errors are followed up, logged, investigated and resolved appropriately on our electronic system PASS.
* Carry out a joint clinical and operational visit quarterly and document this in a visit report. Follow up appropriately on any concerns identified.
* Complete regular monthly contact with the client as agreed, either face to face, phone calls, email or via text message as agreed with the client.
* To review care plans as per clinical need for the client, but as a minimum annually as per CQC guidelines.
* Monitor and action the timely reassessment of worker clinical competencies, ensuring these are kept up to date on People Planner so as not to disrupt the smooth scheduling of staff.
Other managerial and leadership tasks:
* To review and develop policies.
* To identify a complex need specialism within the clinical team and to be the link nurse.
* Attend meetings as required (for example as required by the OM, Multidisciplinary meetings and safeguarding meetings).
* Leading by example, creating a positive, hardworking, results-oriented, can-do culture.
Working relationships:
* Build and maintain effective working relationships with key members across the business, including the OM, CCMs, other CNSs in Bespoke and support workers.
* To develop and maintain monthly reviews with the relevant ICBs.
* Cross work with other areas of the wider business including Team Brain Injury and Team Learning Disabilities and Autism.
CPD:
* Proactively identify areas/opportunities for your own continuing professional development and discuss these with your line manager.

Must have worked with ICU/Spinal or Complex:
* Extensive experience working in Intensive Care Units (ICU), spinal care units, or with complex care patients, demonstrating a high level of expertise in handling severe and multifaceted medical conditions.
* Ability to provide specialised care and manage complex cases, including advanced procedures and multi-disciplinary interventions.
Ability to manage own diary:
* Strong organisational skills with the capability to effectively schedule and manage appointments, patient visits, and other professional commitments independently.
* Proficiency in using diary management tools and techniques to ensure efficient time management and prioritisation of tasks.
Full UK driving license and access to own vehicle:
* Possession of a valid UK driving license and access to a reliable vehicle for travel between locations.
* Ability to transport yourself and others as required for patient visits, meetings, and other job-related duties.
* Flexibility and willingness to travel up to 2 hours within the specified regions to provide care, attend meetings, and engage in other professional activities.
* Preparedness to manage travel logistics and ensure timely arrival at various locations.
Flexible:
* Adaptable and open to changes in work schedules, patient needs, and operational requirements.
* Willingness to work outside of standard hours or take on additional responsibilities as needed to meet patient and organisational needs.
Adult & Paediatric experience:
* Experience in providing care to both adult and paediatric patients, showcasing the ability to address the unique needs of different age groups.
* Comprehensive knowledge of age-specific care practices, treatment plans, and communication strategies for effective patient management.
