Job summary In recent service reviews, an additional band 6 post has been added to the ever-growing department in reflection of the value and need for increased occupational therapy provision. Our static Frailty band 6 post is in a dynamic mixture of rehabilitation and fast paced acute work across two sites that makes this an ideal position to broaden expertise in older adults and frailty for those looking to specialise in this area. The post moves between Same Day Emergency Care (SDEC) Acute Frailty and Frailty Rehabilitation on a cycle of every 7-9 months. We offer a supportive, rewarding and challenging clinical environment. There is an established system for the delivery of clinical governance objectives, CPD and Evidence Based Practice across the service. CPD is encouraged through the Trust appraisal system, as well as through Team CPD Cycles. We encourage journal clubs, formal teaching, skills training and service development, with expectations to lead and participate in these sessions. The post holder requires a flexible approach to working, including cross sites and be able to undertake weekend working commitments as part of a 7 day working rota. Main duties of the job 1. To identify, prioritise and manage patients living with frailty presenting with impairments for Occupational Therapy intervention. 2. To assess and treat patients with complex frailty needs who would benefit from occupational therapy 3. To be competent in the administration of specific Occupational Therapy assessments of motor, sensory, cognitive, perceptual, psychosocial and functional areas. 4. To be competent in the development and implementation of remedial and preventative treatment programmes addressing motor, sensory, cognitive, perceptual, psychosocial and functional areas, both individually and in group settings. 5. To work closely with patients and carers to incorporate service users views in the setting of goals, implementation and evaluation of treatment programmes. 6. To conduct risk assessments and implement risk management strategies including the use of Therapeutic Manual Handling techniques and the Lone Working Policy. 7. To assess and demonstrate to clients the safe use of equipment and adaptations and recommend provision thereof. 8. To write Occupational Therapy reports for referring hospitals, Social Services and other agencies detailing functional status, discharge recommendations, and recommendations regarding ongoing Occupational Therapy intervention. 9. To liaise and work with Social Services and other agencies to ensure continuity of care.
