As a Reablement Officer you will be part of the team providing integrated, multidisciplinary programmes of Reablement across the borough. We are part of Urgent Care which is a multidisciplinary team consisting of Reablement, Therapists, Social Care Workers, Housing and Health professionals. The Reablement service operates 7 days a week, 365 days a year to include all Bank Holidays operating between the hours 07:00 through to 22:00. This role can exceed normal hours of work in line with the service needs. You will work one weekend in four and your hours of work will be 09:30 17:30 About you: You must be creative and self-motivating with a strong customer care ethic. You will be able to demonstrate good interpersonal and communication skills and the value of promoting independence. You will be the first face to face contact from the Reablement service in the person s home. You will use an asset-based approach to promote independence, helping people to remain within their own homes and also connect within their communities to enhance their well-being. You will be required to hold a current valid driving licence and the ability to travel as required to meet the needs of the service. The post will support the existing team to take referrals and will be responsible for assessing, monitoring and reviewing Reablement programmes to include equipment and adaptations. The successful candidates will also have experience of working with people who may require support with medication and moving and handling. You will need to hold: Adult Lead Level 3 in Health & Social Care or have already commenced or demonstrate a commitment to undertake in the first 6 months along with the willingness to obtain the Care Certificate. Recognised certificate in Trusted Assessor or willingness to obtain. Medication Level 2 certificate. This post will require an enhanced DBS. The service can be provided for up to six weeks. The team will support people who have just left hospital or following significant changes in a person s ability to thrive at home.
